Missouri Doctor - Pediatric (Under 18) FREE State Portal Assistance ($59): Pediatric doctor recommendation for Missouri for patients 18 and under. Recommendation for MMJ for state allotment. Patient must have birth certificate and something from a prior physician or medical professional with patients' diagnosis that meet the state qualifying conditions. *These documents must be uploaded prior to the doctor visit.
What documents do I need to provide for the Full-Service?
You'll need to upload:
